basic service to receive premium channels. Additional installation,
equipment, and change of service charges and certain other restrictions

Installation
and Service Charges
The subscriber is
required to maintain basic telephone service or pay a $10.00 monthly charge
for eligibility of Cable TV service. Upon acceptance of these Terms and
Conditions SMTA will provide basic hookup for one TV any additional TVs are
subject to further installation charges. A monthly fee of $4.95 will apply
for each additional cable box provided.
Equipment remains
the property of SMTA.
An early
termination charge of $200.00 will apply to subscribers terminating basic
service prior to one year. Subscribers are required to maintain service for
at least one year.
Upon
cancellation, equipment must be returned in working order, to the office
within 7 days. An additional charge of $200.00 will be charged to the
subscriber for each Cable box not returned to the office within 7 days of
terminating service.
Additional
charges will be applied to the subscriber’s account if SMTA must make a
visit to remove cable equipment.
